• Payment is accepted in cash or check at the end of each session.

• I am considered an out-of-network provider by eligible insurance companies.

• Please check with your insurance company to ensure out-of-network coverage prior to the beginning of services. I will provide you with my professional and business information when you call your insurance carrier for pre-authorization.

• I provide the bill statement at the first appointment of each month for the services provided in the previous month for insurance reimbursement.

​

• My service is covered by a health savings account (HSA) upon prior verification.
